The Continuum of Care programs for homeless assistance fund projects that will assist homeless persons to move to self-sufficiency and permanent housing. The development of a Continuum of Care program is part of the community's larger effort to develop a Consolidated Plan.
The U.S. Department of Housing and Urban Development (HUD) recognizes that many factors, such as mental illness, domestic violence and substance abuse, lead to homelessness. The CoC uses a coordinated community-based effort to address these issues. The components of the CoC process are: outreach and assessment; emergency shelter; transitional housing with supportive service; and permanent housing.
Working with local governments and community groups, HUD's CoC process maximizes resources to fight homelessness through counseling, rehabilitation and assistance. The program helps homeless individuals and families make the transition from the streets to stable lives.
